Your swing plane might be too steep. To much of an up angle is just as bad as swinging down. Some cause might be not regaining your balance point after your stride which results in too much weight on the back leg which can cause the backside to collapse and result in an overly steep approach to the pitch. Try putting more weight on the balls/inside of your front foot prior to heel plant. You don't want to lunge, just have some more weight forward prior to your hips opening. Also, make sure your top hand is not wrapping around the bat so that your knuckles are not aligned properly. Keep a loose grip with the top hand, otherwise you develop some awkward wrist angles as you approach contact. How are you connecting with breaking balls? Is this problem just on fastballs?
