Focus less on your hands and instead think of turning your lead shoulder slightly down and back. This will move your hands back without triggering the swing "instinct". As far as your elbow slotting early, way too early, try some BP with your front heel down, back heel up, both feet pointing towards the pitcher and hips open. Then counter-rotate your shoulders, which moves your hands back, and then slot your back elbow while you swing. Your elbow really doesn't slot until after heel plant and after the hips have opened.
