I've been having the same problem. Though I hvan't been able to consistently correct it, I believe it is also related to the pitches we're chosing. Try forcing yourself to look for low pitches and outside pitches. Film your at bats - are you swinging at high/inside pitches? (Keeping your elbows tight should help with that.)
