still popping up

I made a post several days ago about how I brought my back elbow in and it caused me to pop out twice. Today I popped up two more times. The second one was on a very, very, very good pitch to hit and I am becoming very concerned. What could be causing this?

Bye the way, I took Joe's advice and brought my elbow in to my side at the beggining of my swing also.
