How much does the lead shoulder rotation differ for outside and inside pitches?

What muscles or joints are responsible for lead shoulder rotation?

Do MLB hitters decide to swing from a fully uncommitted position - hands and weight back - or do they "start" the swing on every pitch and then "stop" if they don't like the speed or location?
