Hi Joe,

Taking your word that he has a perfect swing, eye his lead elbow. He may be uppercutting. This is to say that BOTH his elbows (therefore his hands) take a path under the ball, which causes the bathead to follow a much too low path to the ball. (golfswing)

To fix this, make sure his rear elbow is slotted correctly (down at his side) in the swing. The front elbow must work upward (correct upswing). This causes the hands to be above the bathead and above the ball creating a more direct path to the ball.
