Fouling into my foot

Hi, this may simply be a string of stupid luck, but I have hit my foot after a near miss swing (and a few fairly solid hits) every single batting practice for a month now. Not to mention that I almost broke my foot once, I am concerned this is the result of a bad practice. Is my stance just too low and wide or is there another theory? Or bad luck (this never happened once as a kid/teenager in all my years).
