Initial Top Hand Position

What determines the top hand position (ie Knuckle alignment)in relation with the bottom hand during the batter's stance? It appears to me that the elbow height of the top hand is the determining factor. Lower the elbow, and the knuckles form a flat plane. Raise the elbow, and the top hand knuckles bisect the bottom hands' knuckles.
