Weak Grounders

My son is 12 years old and has never, never been a good hitter since the age of nine. He hits weak grounders to pitcher and third base about 80% of the time. I don't think he is rolling his wrists, it appears he's hitting the top of the ball or hitting the ball too late in the swing. He swings hard but it never seems to go anywhere. He's gone to 3 hitting coaches over a four year period and hits great while with them. Also he hits hard when we are practicing. It's gametime he's bad. Any advice.
